- Will be developed with you,and the people important to you to reflect your identified goals.
- Can be flexible to suit the client, eg task trade offs.
- Will be altered as required to reflect any change in support.
- Will state days and times Support Worker will visit.
- Will identify the Registered Nurse who will co-ordinate your support.
- Will reflect TARGET Goals.
- Covers a wide range of support tasks including:
- Hygiene assistance(bathing,showering,hair washing).
- Dressing and undressing support and assistance
- Toileting and assistance with hygiene aids/products
- Grooming support(Face,teeth,hair,shave).
- Medication supervision (support with blister pack medications,eye drops).
- Support hose/TED stocking application.
- Complex cares depending on needs assessed and support worker training.
- Some of OUR SW's have now been trained to INSTILL EYEDROPS. This is only if assessed as competent by and RN, and if advised by an RN - and if it is on the clients support plan, with a signing sheet. Under the indirect supervision of an RN.
